This CLoA confirms that I have chosen to move my Telephony services to a new Provider and request that all telephone numbers associated with these services are ported to the new Provider detailed above. I authorise my new Provider to act on my behalf in relation to this porting request. You have my permission to disclose to them any information required to complete the port, such as site or billing address postcodes, DDI number ranges, Main Billing Number (MBN), or any other relevant service details. I understand that it is my responsibility to arrange the cessation or amendment of any additional services currently supplied by my existing Provider.
